Creatine for Women: What the Research Shows

Creatine is one of the substances that has been widely studied in sports nutrition - but a large part of this research comes from studies with male test subjects. In a review article in the Journal of the International Society of Sports Nutrition (2025), a team of authors led by Abbie Smith-Ryan expressly states that data on women in individual phases of life are missing and are considered a research priority. Much of what you read about creatine in women is based on translations from men's studies or on hypotheses. Here we separate the proven from the suspected.

What creatine does in the body

Your body stores creatine predominantly in the skeletal muscles, according to the PTA forum about two thirds as creatine phosphate and one third as free creatine. Creatine phosphate serves as a quickly available energy buffer for short, intense exertion - i.e. for your sets in the weight room, for sprints and intervals, not for long endurance runs.

You get part of what you need through food. The consumer advice center states that fish and meat contain around 0.5 grams of creatine per 100 grams; Cow's milk is around 10 milligrams per 100 grams. Do the math: If you wanted to consume the 3 grams per day required by the approved EU health claim from fish or meat alone, you would calculate around 600 grams per day. If you eat vegetarian or vegan, this option is largely unnecessary.

Creatine storage in women: the numbers in detail

The much-quoted statement that women have significantly lower creatine stores goes back to the review by Smith-Ryan and colleagues in the scientific journal Nutrients (2021). It says that women have 70 to 80 percent lower body creatine stores than men. What is often overlooked when telling others: The same study reports that the creatine concentration in resting muscles is about 10 percent higher in women than in men.

The more recent review from 2025 quantifies the differences differently and refers to reports of around 20 percent lower endogenous synthesis rates and an average of 30 to 40 percent lower creatine intake through food. So it's better to read the general statement of 70 to 80 percent with caution - it doesn't simply describe "the muscle is emptier".

Strength and training performance: what the studies show

There are two approved health claims in the EU for the performance effect. The first, according to the PTA forum, states that creatine increases physical performance during speed training as part of short-term intensive physical activity; The requirement for this is a daily intake of 3 grams for adults who train intensively. The second relates to people over 55 and that daily creatine intake can increase the effect of resistance training on muscle strength. Without training, neither works - creatine does not replace training stimulus.

The 2021 review paper reports a ten-week training study in which the creatine group achieved 20 to 25 percent greater gains in the one-repetition maximum in the leg press, leg extension and squat than the placebo group. More widely supported is a systematic review with meta-analysis by Naddafha and colleagues (2026), which summarizes seven randomized controlled trials with 608 postmenopausal participants; the mean study duration was 38 weeks. An increase in fat-free mass of 0.37 kilograms (95 percent confidence interval 0.05 to 0.69 kg) and a 7.5 kilogram higher performance on the leg press (confidence interval 2.2 to 12.8 kg) compared to placebo were reported. Overall, bone density remained unchanged. The effects were seen when at least 5 grams daily were combined with strength training; lower amounts without training did not produce a significant difference.

These are real but moderate numbers. An increase of 0.37 kilograms of fat-free mass over a study period of an average of 38 weeks is not a change in body composition, but rather a small additional effect on training that you are already doing.

Cycle and menopause: young and inconsistent study situation

There are a lot of promises circulating around the cycle and menopause. Research doesn't give it that way. The 2025 review points to animal models in which creatine kinase activity follows fluctuations in estrogen levels; Reliable data from studies on humans are limited. A plausible hypothesis can be derived from this, but no application recommendation.

The situation is clear for perimenopause as a phase of life: to the knowledge of the same team of authors, no study has so far directly examined the effect of creatine intake in perimenopausal women. Reliable data is available for the period after menopause, and there it relates to muscle mass and strength in combination with strength training - not to sensitivities.

Weight gain: what actually happens

A recurring point in the discussion concerns the scale. The consumer advice center describes the mechanism soberly: Creatine causes increased water retention in the muscle cells. So the extra weight is water, not body fat. The 2021 review classifies such an increase as an expression of increased cell hydration; In the studies evaluated there, body weight and body fat percentage did not differ significantly between the groups.

A finding from the work from 2025 is interesting: In a reported study, the amount of intracellular fluid increased in the luteal phase with creatine intake without any change in body weight. Water in the muscle and “acting bloated” are not the same thing physiologically – a point that is often lost in the discussion about Creatine products.

Formats in everyday life

Creatine monohydrate is the form to which the studies cited predominantly refer. The format is then all about practicality, not effectiveness - use what you can reliably do in everyday life.

Format What that means in everyday life
Powder in a can You are free to choose the portion size - but you need a spoon or scale and a drink to mix.
Capsules Without taste and easy to transport; How many pieces make up a daily portion is determined by the amount of creatine per capsule.
Portion sticks Pre-dosed, meaning there is no need to measure and is practical for on the go or when traveling.

About the quantity: The consumer advice center states that a maximum daily amount of 3 grams is acceptable for healthy adults from a safety perspective and points out that the sports nutrition working group of the German Nutrition Society considers up to 5 grams of creatine monohydrate daily to be safe for athletes. The 2021 review also describes loading phases of 20 grams daily for about five days, followed by 3 to 5 grams for maintenance; Alternatively, 5 grams daily for around three to four weeks are mentioned there. This is referenced study information, not a recommendation - the dosage information on the package is important for you.

A look beyond training

At the Research Center Jülich, a group led by Ali Gordjinejad investigated the effects of a single high dose of creatine during a sleepless night. According to the center's press release, an effect appeared from the third hour after ingestion, peaked after four hours and lasted up to nine hours; Processing performance and short-term memory in particular have improved. Important for classification: 15 people took part in the study. This is a signal for further research, not a basis for everyday recommendations.

When you can save on creatine

Now the honest part: Without regular, demanding training, little can be expected from creatine intake - the approved claims are linked to exactly this. The consumer advice center therefore classifies creatine as only helpful in rare cases. If you train rarely or without peak loads, you are better served with regular training, sleep and a sufficient supply of protein. And if you want to get started with a manageable scope, Starter sets suit you better than a bulk pack.

To be on the safe side: According to the consumer advice center, people with previous illnesses, pregnant women, breastfeeding women, as well as children and adolescents should only take creatine after a doctor's recommendation. In the 2026 meta-analysis, adverse events were described as mild and comparable to placebo.

Frequently asked questions

Does creatine work differently on women than on men?

The review from 2025 describes differences in the storage, metabolism and utilization of creatine, including reports of around 20 percent lower endogenous synthesis rates. However, the approved EU health claim for physical performance during high-speed exercise does not differentiate between gender and is linked to 3 grams daily.

Do you need a loading phase?

The review from 2021 describes loading phases of 20 grams per day over around five days, as well as a continuous intake of 5 grams over around three to four weeks. The meta-analysis of postmenopausal women showed effects from at least 5 grams daily in combination with strength training. The dosage information for your product remains decisive.

Why does the weight increase in the first few weeks?

According to consumer advice centers, creatine stores more water in the muscle cells. The 2021 review classifies such an increase as cell hydration; Body weight and body fat percentage did not differ significantly between the groups in the studies evaluated.

What is proven about menopause and the cycle?

There is data on muscle mass and strength in connection with strength training for the period after menopause. Regarding perimenopause, the authors of the 2025 review note that they are not aware of any studies on this phase of life. Further statements are currently not supported by studies.

By the way: If you buy your basics as a package, you will find ready-made sets in the Starter sets and Professional sets - or stock up for longer: as 30-day , 90-day or year supply.
